Overview

A part of this program will use theatrical devices to tell the story of how palliative medicine is supportive of curative care in any medical setting with patients living with serious illnesses.  In the context of a growing population of people of all ages living for many years with serious chronic conditions, spread of new care models and new skills organized around the dominant needs of this group are necessary.  In the last decade, accumulating evidence demonstrates the value of palliative care approaches in terms of both improved quality and satisfaction, and greater efficiency of care .


Objectives
At the conclusion of this activity, learners will be able to:

  1. Define high quality primary palliative care as a focus on maximizing quality of life focused care concordant with patient goals.
  2. Engage with patients facing serious illness using advanced communication techniques.
  3. Incorporate and use NURSE (VITAL Talk Communication Tool).
  4. Utilize the VITAL Talk Communication Tool when asking patients abou their understanding of their illness.
  5. Utilize the VITAL Talk Communication Tool when asking patients about what is most important to them.

  • Yale Residents/Fellows/Students – complimentary, registration required

This conference is being held at Yale West Campus Conference Center, 800 West Campus Drive, Orange, CT.

Coming from the South:
If you are coming from the South (New York), take I-95 North and get off at Exit 41 (Marsh Hill Rd). At the end of the ramp, go straight through the light. This is West Campus Drive and the entrance ramp to I-95 Northbound. Stay in the right lane and bear right at the fork.  Follow signs to building 800- Conference Center.  
Coming from the North:
If you are coming from the North (Hartford/New Haven/Madison, etc.), take I-95 South. Get off at Exit 41 (Marsh Hill Rd). At the light at the end of the exit ramp take a left onto Marsh Hill Road. At the next traffic light, take a left onto the West Campus Drive/I-95 Northbound entrance ramp. Follow this to the fork, stay in the right lane.  Follow signs to building 800- Conference Center. 
Coming from the Merritt Parkway:
Take the Merritt Parkway North to Exit 54 (Milford Connector). Go under the I-95 overpass and exit onto the ramp designated to New Haven (I-95 North). Get off at Exit 41 and continue with the directions above (Coming from the South) to reach West Campus.

The Yale University Shuttle provides transportation for the Yale community between the West Campus and the Central and Medical School campuses. If you are taking a shuttle, please inform the driver you are going to Yale West Campus, building 800 (The Conference Center). 
  
The Green line runs Monday - Friday and travels directly between the West Campus and Science Hill, Lot 22. The Purple line runs 7 days per week and travels directly between the West Campus and the Medical School and also to Union Station.
